【AAAC高齢状態コラム038】地域にも孤立があります。
「孤立」は、一人暮らしの人だけの問題ではありません。
地域そのものが孤立していることがあります。
高齢者が地域へ出る場所がない。
相談できる人がいない。
支え合う仕組みがない。
医療・介護・行政・住民がつながっていない。
こうした状態は、地域全体の「孤立」です。
AAACでは、人だけではなく、家族、そして地域も「状態」として評価します。
地域の状態が良ければ、高齢者も家族も安心して暮らせます。
しかし地域が孤立すると、支援は遅れ、介護は重くなり、家族の負担も増えていきます。
だから私たちは、
「地域にはどんな人がいるか」ではなく、
「地域はつながっているか」を見ます。
Connection(つながり)は、人だけではなく地域全体に存在します。
高齢社会では、
「地域の孤立」を見つけることが、
これからの支援の第一歩になります。
AAACは、
本人・家族・地域という三つの状態を理解し、
誰も孤立しない地域づくりを目指しています。

Isolation is not only an issue for individuals.
Communities themselves can become isolated.
There may be no place for older adults to gather.
No one to ask for help.
No network connecting healthcare, long-term care, local government, and residents.
AAAC evaluates not only individuals but also families and communities as "states."
A connected community supports better aging.
An isolated community increases risks, caregiver burden, and delayed support.
Therefore, AAAC asks:
Is your community truly connected?
Connection is one of the most important foundations of a well-aging society.

孤立,并不仅仅发生在老人个人身上。
社区本身,也可能处于孤立状态。
没有老人可以去的地方;
没有可以商量的人;
医疗、护理、政府、居民之间缺乏连接。
AAAC不仅评估老年人,也评估家庭和社区的状态。
社区连接越强,
老人的生活越稳定。
真正需要关注的,不只是个人是否孤独,
更是整个社区是否彼此连接。
AAAC希望建设一个没有孤立的社区。
