転ばないように歩かせない。
迷わないように外へ出さない。
事故を防ぐために、本人の行動を制限する。
介護では「安全のため」という言葉が、本人の自由を奪う理由になることがあります。
もちろん、安全を軽視してよいわけではありません。しかし、危険を完全になくそうとすれば、本人が動く、選ぶ、挑戦する機会まで失われます。身体機能だけでなく、意欲や役割、その人らしい生活も衰えていくかもしれません。
尊厳介護に必要なのは、安全か自由かの二者択一ではありません。
本人は何をしたいのか。
どのような危険があるのか。
環境や支援方法を変えられないか。
本人と家族は、その危険をどう考えるのか。
これらを対話によって確認し、危険を減らしながら、本人が望む生活をできる限り守ることです。
介護者が責任を避けるための安全ではなく、本人が安心して自由を使うための安全をつくる。
「危ないから禁止する」の前に、「どうすれば実現できるか」を考える。
自由を奪わない安全の設計が、尊厳を守る介護につながります。

Are We Taking Away Freedom in the Name of Safety?
We do not let a person walk because they might fall.
We do not let them go outside because they might get lost.
We restrict their actions because we want to prevent accidents.
In caregiving, the phrase “for your safety” can become a reason to take away a person’s freedom.
Safety must never be ignored. However, if we try to eliminate every possible risk, we may also eliminate opportunities to move, choose, try, and participate in life. The person may gradually lose not only physical abilities but also motivation, social roles, and a sense of self.
Dignity care is not a choice between safety and freedom.
What does the person want to do?
What specific risks are present?
Can the environment or support method be changed?
How do the person and their family understand the risks?
Through dialogue, we reduce preventable danger while protecting the life the person wishes to live.
Safety should not exist merely to protect caregivers or organizations from responsibility. It should enable the person to use their freedom with greater confidence.
Before saying, “It is dangerous, so you cannot do it,” we should ask, “How could we make it possible?”
Designing safety without taking away freedom is part of protecting human dignity.

是否以安全为由,剥夺了老年人的自由?
因为担心跌倒,所以不让他走路。
因为担心迷路,所以不让他外出。
因为害怕发生事故,所以限制他的行动。
在照护现场,“为了安全”有时会成为剥夺本人自由的理由。
安全当然不能被忽视。但是,如果试图消除所有风险,也可能同时夺走本人行动、选择、尝试和参与生活的机会。随之衰退的可能不仅是身体能力,还有生活意愿、社会角色和自我价值感。
尊严照护不是在安全与自由之间二选一。
本人想做什么?
具体存在哪些风险?
能否调整环境和支持方法?
本人和家属如何理解这些风险?
照护者应当通过对话进行确认,在降低可以预防的风险的同时,尽可能保护本人希望过的生活。
安全不应只是为了让照护者和机构回避责任,而应当帮助本人更安心地运用自己的自由。
在说“太危险,所以不可以”之前,先思考:“怎样才能帮助他实现?”
不以牺牲自由为代价来设计安全,才是维护尊严的照护。
