【高齢社会探訪記040】してあげる介護から、できるを支える介護へ

介護では、「危ないから」「時間がかかるから」という理由で、介護者が先に手を出してしまうことがあります。 着替えさせてあげる。食べさせてあげる。移動させてあげる。 それは親切に見えます。しかし、本人ができることまで代わってしまえば、身体を動かす機会だけでなく、考えること、選ぶこと、決めることも奪いかねません。 尊厳介護が目指すのは、何でも自分でさせることではありません。本人の状態を見極め、できる部分は待ち、難しい部分だけを支えることです。 「どこまでできますか」
「何を手伝いましょうか」
「一緒にやってみませんか」 この対話によって、介護は一方的な援助から、本人との共同作業へ変わります。 重要なのは、介護者がどれだけ多くのことをしたかではありません。その人が自分の力を使い、自分の意思で生活に参加できたかです。 できないことを補いながら、できることを見つけ、その力が続くように支える。 「してあげる」から「できるを支える」へ。 そこに、その人の自律と尊厳を守る介護があります。 高齢社会探訪|尊厳介護を歩く

From “Doing It for Them” to Supporting What They Can Do In caregiving, caregivers sometimes step in too quickly because an action seems dangerous or takes too much time. They dress the person, feed them, or move them without first seeing what the person can still do. This may look caring. However, when we take over even the things a person can do, we may deprive them of opportunities to move, think, choose, and make decisions. Dignity care does not mean forcing people to do everything independently. It means understanding their condition, waiting when they can act for themselves, and assisting only where support is genuinely needed. “How much can you do yourself?”
“What would you like help with?”
“Shall we try it together?” Through this dialogue, caregiving changes from one-sided assistance into a shared activity. What matters is not how much the caregiver has done. What matters is whether the person has used their own abilities and participated in daily life through their own choices. We compensate for what is difficult, discover what remains possible, and support that ability so it can continue. 从“替他做”的照护,转向“支持他能做”的照护 在照护现场,照护者有时会因为“太危险”或“太费时间”,还没有确认老年人能做什么,就先伸手替他完成。 替他穿衣、替他吃饭、替他移动。 这些行为看起来是体贴和帮助。但是,如果连本人能够完成的事情也全部代替,就可能剥夺他活动身体、思考、选择和决定的机会。 尊严照护并不是要求老年人什么都必须自己做,而是准确了解本人的状态。能够自己完成的部分,照护者应当耐心等待;确实困难的部分,再给予适当支持。 “您能自己做到哪一步?”
“您希望我帮助什么?”
“我们一起试一试,好吗?” 通过这样的对话,照护将从单方面的帮助,转变为照护者与本人共同完成的生活实践。 重要的不是照护者做了多少,而是本人是否运用了自己的能力,是否按照自己的意愿参与了生活。 弥补他做不到的部分,发现他仍然能做的事情,并支持这种能力继续保持。 从“替他做”,转向“支持他能做”。 这正是维护本人自主与尊严的照护。 老龄社会探访|行走在尊严照护之路
