高齢期の衰えは、以前から「虚弱」「老衰」などの言葉で捉えられてきました。しかし、それでは「年齢による避けられない変化」という印象が強く、早期に気づき、働きかける視点が十分ではありませんでした。
現代のフレイル研究を大きく進めた一つが、2001年にFriedらが示した身体的フレイルの考え方です。体重減少、疲労感、活動量低下、歩行速度低下、筋力低下という五つの特徴から状態を捉えました。
一方、Rockwoodらは、病気、症状、障害、生活上の問題などがどの程度蓄積しているかを「フレイル・インデックス」として捉えました。
日本では2014年、日本老年医学会が「虚弱」に代わる言葉として「フレイル」を提唱しました。
重要なのは、フレイルを要介護へ向かう一方向の衰えではなく、適切な支援によって改善する可能性のある状態として捉え直したことです。
ただし、フレイルは筋力だけの問題ではありません。心理、認知、食事、口腔、外出、人とのつながり、社会的役割も相互に影響します。
フレイルの歴史は、「年齢だから仕方がない」から、「小さな変化に気づき、生活を整える」への転換の歴史です。

Exploring the Aging Society|From Frailty to Well-Aging
Age-related decline has long been described using words such as weakness and senility. These expressions, however, often suggested that decline was an unavoidable consequence of aging.
A major development in modern frailty research came in 2001, when Fried and colleagues proposed a physical frailty phenotype based on five characteristics:
* Unintentional weight loss
* Exhaustion
* Low physical activity
* Slow walking speed
* Reduced muscle strength
Rockwood and colleagues developed another approach: the Frailty Index. It considers how symptoms, diseases, disabilities, and other health-related deficits accumulate within a person.
In Japan, the Japanese Geriatrics Society introduced the term “frail” in 2014 as an alternative to the Japanese word traditionally used for weakness.
The important shift was to view frailty not simply as an irreversible path toward disability or long-term care, but as a condition in which appropriate support may help prevent further decline and, in some cases, improve the person’s condition.
Frailty is not only about muscle strength. Physical, psychological, cognitive, nutritional, oral, and social factors interact with one another.
